  1. Valid, signed US passport with blank pages for visa and valid 6 months beyond trip. If your passport does not meet these requirements, see our passport services.
     
  2. One Visa Application Form completed and signed.
  3. One Visa Connection Cover Sheet.  
     
  4. Two passport photographs.
     
  5. Tourist voucher from hotel in Azerbaijan that list the confirmation number
     

Fees and Processing Time:

• Consular fee and processing time:
  
  $ 40 Per Visa for 4 days processing time/ single
  $ 80 Per Visa for 4 business days processing time/ double
  $ 250 Per Visa for 4 business days processing time/ multiple
   
• Visa Connection fee: 

  $ 75.00 per visa (add 20.00 for rush). 

   

Shipping fee:

• Shipping Fee

  Add 25.00 to the service fee OR include a pre-paid FEDEX air bill for the overnight return of your passport.
   

  Payment:

  • Include a check or money order payable to Visa Connection or put your credit card information where requested on the Visa Connection Cover Sheet.
