- 3. Four Passport photographs.
- 4. One Visa Connection Cover Sheet.
- 5. International Certificate of Vaccination for Yellow Fever.
- 6. Itinerary.
- 7. Copy of recent bank statement or letter from bank verifying applicant has sufficient funds.

- 6. Copy of round trip airline tickets or itinerary, or letter of confirmation from travel agent.
7. For multiple entries visa, a notarized letter of invitation from the host in Cameroon is required.
- 8. A business letter of responsibility is required, to be typewritten on your company letterhead explaining Nature of business to be performed in Cameroon, Name and address of reference to be visited in Cameroon, Guarantee of return transportation, and sufficient funds for the visit. The letter should be signed by an officer of the company other than the applicant.

$275.00 Per Visa for 6 months Multiple Entries (Business only). First entry must be made within 90 days of visa issue. Stay up to 30 days each visit. Extension of stay possible on local application.

Include a check or money order payable to U.S. Visa Connection or put your credit
card information where requested on the Visa
Connection Cover Sheet.
