Passport Agency Instruction:
Additional Passport Pages

Note: If the visa pages in your passport are full, then you can request the addition of passport pages. Keep in mind that the pages in rear of the passport marked "amendments and endorsements" are not intended for visa or entry stamps.

STEP 1:
Complete and Submit Form DS-4085
Application For Additonal Passport Pages

(Complete the Online Passport Form DS-4085, then click CREATE FORM button)


If for any reason you cannot complete the online passport application,
print the DS-4085 Passport handfill Application and fill out by hand.

STEP 2:
Submit Current Passport

You must submit your current passport to acquire additional passport pages.
Note: If your passport is damaged, you will be required to apply for a new passport.

 

STEP 3:
Complete and Submit Two Letters of Passport
Authorization

Print and complete two copies of the Passport Authorization Letter

 

STEP 4:
Complete and Submit an Passport Order Form

Print and complete the Passport Order Form

 

 

STEP 5:
Submit Proof of Travel

• Copy of Itinerary or Ticket OR
• Company Letter (example) stating your job related need for travel
  
  Note: If you cannot provide proof of travel, your passport will be processed in
  5-10 business days.
  
  
  

 

STEP 6:
Passport Fees

Government Fees:

Make one check or money order to "US Department of State" for $142.00 (this is the government passport processing fee).

Expediting Service Fees:
Include a check or money order payable to U.S. Visa Connection or put your credit card information where requested on the Passport Order Form.

Same day - 199.00 + return shipping

24-48 hours - $149.00 + return shipping

5-10 day - 99.00 + return shipping
