Passport Agency Instruction:
Passport Renewal

You are applying for a passport renewal if:

You still have and can submit your most recent passport. You were over the age of 16 when the passport was issued. You received your most recent passport within 15 years (has not been expired more than 5 years) The current passport is undamaged

STEP 1:
Complete and Submit Form DS-82
Application For A U.S. Passport Renewal

(Complete the Online Passport Form DS-82, then click CREATE FORM button)


If for any reason you cannot complete the online passport application, print the
DS-82 Passport Handfill Application and fill out by hand.

STEP 2:
Submit Current Passport

You must submit your current passport.
If your passport is damaged, lost/stolen, expired more than 5 years, or was acquired before the age of 16, you must apply for a new passport.

 

STEP 3:
Provide Two Passport Photos

Your Passport Photographs Must Be:

• Identical
• In color
• 2 x 2 inches in size
• Taken within the past 6 months, showing current appearance
• Full face, front view with a plain white or off-white background
• Between 1 inch and 1 3/8 inches from the bottom of the chin to the top of the head
• Taken in normal street attire:
• Uniforms should not be worn in photographs except religious attire that is worn daily
• Do not wear a hat or headgear that obscures the hair or hairline
• If you normally wear prescription glasses, a hearing device, wig or similar articles, they should be worn for your picture
• Dark glasses or nonprescription glasses with tinted lenses are not acceptable unless you need them for medical reasons (a medical certificate may be required)
  
  
  
  

 

STEP 4:
Complete and Submit Two Letters of Passport
Authorization

Print and complete two copies of the Passport Authorization Letter

 

STEP 5:
Complete and Submit an Passport Order Form

Print and complete the Passport Order Form

 

STEP 6:
Submit Proof of Travel

• Copy of Itinerary or Ticket OR
• Company Letter (example) stating your job related need for travel
  
  Note: If you cannot provide proof of travel, your passport will be processed in

STEP 8:
Passport Fees

Government Fees:

Make one check or money order to "US Department of State" for $170.00 (this is the government passport processing fee).

Expediting Service Fees:
Include a check or money order payable to U.S. Visa Connection or put your credit card information where requested on the Passport Order Form.

5-10 day - 99.00 + return shipping
