STEPS TO SUBMITTING A SECOND U.S. PASSPORT APPLICATION:

 

If you travel extensively and have a schedule conflict, you may be eligible to obtain a second passport.

 

STEP 1: Complete and Submit Form DS-82: Application For A U.S. Passport Renewal

(Complete the Online Passport Form DS-82, then click CREATE FORM button)

If for any reason you cannot complete the online application, print the DS-82 Passport Application
and fill out by hand.

STEP 2: Submit Current Passport

You must submit your current passport.

 

STEP 3: Provide Two Passport Photos

Your Passport Photographs Must Be:

• Identical
• In color
• 2 x 2 inches in size
• Taken within the past 6 months, showing current appearance
• Full face, front view with a plain white or off-white background
• Between 1 inch and 1 3/8 inches from the bottom of the chin to the top of the head
• Taken in normal street attire:
  • Uniforms should not be worn in photographs except religious attire that is worn daily
  • Do not wear a hat or headgear that obscures the hair or hairline
  • If you normally wear prescription glasses, a hearing device, wig or similar articles, they should be worn for your picture
  • Dark glasses or nonprescription glasses with tinted lenses are not acceptable unless you need them for medical reasons (a medical certificate may be required)

 

STEP 4: Complete and Submit Two Letters of Passport Authorization

Print and complete two copies of the Passport Authorization Letter

 

STEP 5: Complete and Submit an Passport Order Form

 

Print and complete the Passport Order Form

 

STEP 6: Submit Proof of Travel

• Copy of Itinerary or Ticket OR

• Comapny Letter stating your job related need for travel
  
  Note: If you cannot provide proof of travel, your passport will be processed in 5-10 business days.

 

STEP 7: Submit a business letter

You must submit a business letter stating one or more of the following:

• Your need of a second passport due to excessive travel

• Your need of a second passport due to scheduling conflicts and visa acquisition

• Your need of a second passport due to visa stamp conflicts

 

STEP 8: Passport Fees

Government Fees:

• Make one check or money order to "US Department of State" for $170.00 (this is the government passport processing fee).
